personally appeared before me, a __Notary Public__ within and for the county and State aforesaid, __George Colling__ who, being duly sworn according to law, declares that he is 67 years of age, and a resident of __Hastings__ county of Adams , State of Nebraska ; and that he is the identical person who was ENROLLED at Blackberry, Kane Co Ill under the name of George Colling , on the 19th day of__ February__ , 1864 as a __private__ , in__Company A 52 Ill Vol. ___Infantry ___________in the service of the United States. in the
__Civil__ war, and was HONORABLY DISCHARGED at Louisville, Kentucky_ on the 6th _day of July__ , 1865__

That he makes this declaration for the purpose of being placed on the pension roll of the United States under the provisions of the act of February 6, 1907.
